Animals in Mind (AiM) believes that no child should be excluded from education because of a medical condition or the treatment it requires. Our policy is designed to ensure that learners are supported and parents feel reassured.
However, to allow us to provide medication to our learners, consent must be provided for each medical condition from a parent, guardian or caregiver. Please complete the below form carefully. You will receive a copy of your medication consent via email upon submission and we shall be in touch once we have reviewed the medical requirements.
